Media Consent Form

This form should be filled out and signed by parishioners that may be photographed or filmed for a parishes facebook, websites or general PR purposes. You may not photograph or film someone without their permission. These forms should be submitted to the parish they attend.

Youth Programming Policy Form 

This form is for anyone leading a youth program, such as sunday school or a summer camp. It should be signed and returned to the parish the program is taking place at.

Participant Form

This form is for participants in youth programming. These forms should be filled out prior to the beginning of each program and kept in a secure place at that parish, when not with the teacher who is leading the program. For the duration of the program, these participant forms should always be in the room where the program is taking place and readily available if needed in the event of an emergency.
